Watch the arrow symbols that appear next to your character during a track. Press the matching arrow keys in rhythm, so Boyfriend hits every note and keeps the health bar from dropping. Miss too many arrows and the opponent wins the round. Some songs swap Boyfriend for other Friday Night Funkin' street singers, so the playable character can change between tracks while the control scheme stays the same.
